Student-Athlete Forms

ATTENTION ATHLETES! -- If you want to participate in any sports at UW-Sheboygan, you must:

  1. Carry at least 9 credits per semester and meet the GPA requirement of 1.75 after first semester and a cumulative GPA of 2.0 for every semester thereafter
  2. Download, Read and Complete the necessary forms below:

    WCC Code of Conduct

    UW-S Code of Conduct

    Physical Form

    Hold Harmless Liability Form

  3. Return all completed forms to either Missy Skurzewski (PE Center #8006) or your sport's coach.
